Foundations Screening Assessments
What Is Foundations?
Foundations is a structured behavioral health screening assessment designed to help families understand patterns in a child’s emotional, developmental, sensory, and behavioral functioning. It is not a diagnostic evaluation or a substitute for therapy — it’s a tool to help guide the right support at the right time.
Families come to Foundations when:
- A child is struggling emotionally or behaviorally, but the cause is unclear
- Teachers or pediatricians have raised concerns but formal testing isn’t yet in place
- A family is unsure whether a child’s needs are developmental, sensory, emotional, or behavioral
- Therapy has started, but a broader picture of functioning is needed to guide care
What Does It Include?
Foundations screening includes:
- Caregiver Interview: A focused conversation to gather relevant history, current concerns, and family context
- Standardized Screenings: A customized selection of evidence-based screening tools exploring behavior, mood, attention, sensory processing, social communication, trauma history, and more
- Summary Report: A clear, easy-to-read written summary with key patterns, clinical impressions, and considerations for care
- Review & Guidance: A meeting to go over the findings, answer questions, and discuss recommendations for next steps
All services are provided by licensed clinicians experienced in child development, trauma, and family systems.
What Ages Can Be Screened?
Foundations is available for children and teens ages 3–17. Screening tools are selected based on age, concerns, and developmental level.
What Can This Help Us Understand?
Foundations can provide clarity on:
- Emotional regulation and behavior
- Attention, focus, and executive functioning
- Social or communication challenges
- Possible trauma impacts
- Sensory sensitivities
- Patterns that may suggest a need for further evaluation for autism, anxiety, OCD, ADHD, or other concerns
The results help guide families toward next steps, whether that’s therapy, occupational therapy, educational support, further evaluation, or simply reassurance.
What Foundations Is Not
Foundations is not a diagnostic evaluation, psychological testing, or a school eligibility assessment. It does not replace formal psychological or developmental evaluations. It is not used for court or forensic purposes. Foundations was designed to guide insight and care planning, not to assign diagnoses.
